COIC's employment application is available in Word and Adobe Acrobat format (PDF). If you have Word, please save the "Employment Application" to your computer, then open it by double clicking to fill it out. You can then tab through the fields to fill out the form.

In order to be considered for a position(s), a completed COIC application must be submitted. Once you have completed the application you will need to print and sign the application. The preferred method for recieving applications is by sending a scanned or emailed copy to the contact below. If you are unable to email the completed application, you can print and fax a copy to (541) 923-3416 or mail it to:
