If you have Word, please save the "Employment Application (Word)" to your computer, then open it by double clicking to fill it out. You can then tab through the fields to fill out the form.

Print the application and mail it to 2363 SW Glacier Pl., Redmond, OR 97756. Faxed applications will be accepted at (541)923-3416 if followed by the signed original. You will not be able to submit applications by e-mail as they must be signed. You may also print the Adobe Acrobat file and fill it out by hand or typewriter. You will not be able to fill out the Acrobat version online.
